ACTIVITY RELEASE

I, the parent/guardian of the above-mentioned child(ren), grant permission for my child(ren) to participate in the following acitivties: Veritatis Splendor Summer Camp. As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the above-named minor participant(s). I agree on behalf of myself, my child(ren) named as minor participant(s) herein, or our heirs, successors, and assigns, to hold harmless and defend Saint Thomas Parish, its officers, directors, employees, and agents, and the Diocese of Covington, its employees and agents, chaperones, or representatives associated with the activities, from any claim arising from or in connection with my child(ren) participating in the activities, or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith, and I agree to compensate Saint Thomas Parish, its officers, directors, and agents, and the Diocese of Covington, its employees and agents and chaperones, or representative associated with the activities for reasonable attorney's fees and expenses which they may incur in any action brought against them as a result of such injury or damage, unless such claim arises from the negligence of Saint Thomas Parish or the Diocese of Covington. With the exception of the above-listed allergies/restrictions, I hereby warrant that to the best of my knowledge, my child is in good health, and I assume all responsibility for the health of my child.

PHOTO RELEASE

For any promotional purposes involving Saint Thomas Parish and its Veritatis Splendor programming, and/or for promotional purposes involving the ministry of Saint Thomas Parish, I, as the custodial parent/guardian of the above named minor child/children, hereby grant consent to use, and release to, Saint Thomas Parish the use of my minor child's/children's photograph(s), whether in still, motion pictures, audio and/or video tape; photographic reproductions of my minor child/children including his/her/their voice (which includes commentary, remarks, and/or recordings); my minor child's/children's features, with or without his/her/their name.
